Position Title: Family Support Advocate

Program(s) and Location: New Direction (Howe Ave)

Schedule: Full-time non-exempt, 40 hours/week, Monday - Friday 8:00 am - 4:30 pm, with potential evening and weekend shifts.

Compensation: $21.87/hour - $22.97/hour based on experience and education.

Position Overview:

The Family Support Advocate will be an integral part of a multi-disciplinary team, delivering peer and family support in collaboration with Personal Service Coordinators. This role focuses on assisting clients in accessing program services and community resources to enhance their well-being.

The Advocate will exemplify personal recovery and coping skills, serving as a role model while helping clients develop their own coping strategies and wellness plans that align with their cultural values. Advocacy for clients' choices and voices is a key component of this position.

Key Responsibilities:

  1. Provide support to clients and family members through active listening, empathy, and fostering a message of hope and recovery.
  2. Collaborate with Personal Service Coordinators, Team Coordinators, Clinicians, and Medical staff to coordinate and deliver services for clients.
  3. Observe, assess, and refer clients regarding mental health symptoms, physical health issues, and substance use concerns.
  4. Assist clients and family members in connecting with community resources as outlined in the client plan.
  5. Help clients create and adhere to personalized wellness plans tailored to their needs.
  6. Support clients in developing independent living skills, including household management, budgeting, and medication management.
  7. Provide transportation assistance as needed.
  8. Engage with clients and family members in various community settings to ensure continuity of care.
  9. Facilitate social, cultural, educational, and recreational group activities.
  10. Complete all required documentation accurately and promptly.
  11. Act as a Mandated Reporter for suspected abuse or neglect of vulnerable populations.

Qualifications:

  • Personal experience as a mental health consumer or as a family member of a mental health consumer.
  • Experience with Medi-Cal billing is preferred.
  • Experience working with individuals facing substance abuse challenges, particularly those with co-occurring disorders, is highly desirable.
  • Familiarity with 12-Step Programs or personal experience in substance abuse recovery is advantageous.

Certificates and Licenses:

  • Possession of a valid California Driver's License and current vehicle insurance.
  • A minimum of 3 years of driving experience is required.
  • CPR and First Aid certification must be obtained within the first six months of employment.
  • Completion of Pro-Act training and certification within the first year of employment is mandatory.

Skills and Requirements:

  • Ability to pass background checks from various agencies.
  • Willingness to work overtime as needed.
  • Availability for after-hours duty on a rotational basis.
