The LPN Nurse - Care Quality Leader plays a crucial role in crafting extraordinary experiences for clients and their families, overseeing the management of top-tier home care plans.

Responsible for developing and maintaining personalized client care plans, ensuring exceptional client experiences while effectively communicating with clients, families, care partners, and referral sources.

Actively engages with clients and families to guarantee safe, appropriate, and meaningful care delivery.

·

Serves as a mentor to caregivers, supporting their care practices.

·


Maintains a composed and positive attitude at all times, demonstrating the ability to multitask in a high-performing office setting.

Exhibits outstanding skills in communication, organization, and decision-making.

The ideal candidate will have prior experience working with elderly patients in home care or similar medical environments, although this is not mandatory.

Preferred qualifications for the LPN Care Quality Leader include:

Unencumbered current New York Practical Nurse license.

·

At least two years of experience as an LPN Nurse.

·

Able to work independently with sound judgment.

·

Availability for on-call duties outside of standard office hours as needed.

·

Possession of a valid driver's license and access to an insured vehicle.

·

Ability to learn and adapt to computer systems.
