The LPN Nurse - Care Quality Supervisor plays a crucial role in crafting exceptional client and family experiences and oversees the management of optimal home care plans.


Key Responsibilities:

  • Develop and uphold client care plans, ensuring outstanding client experiences while effectively communicating with clients, families, care partners, and referral sources.
  • Actively engage with clients and families to guarantee safe, suitable, and meaningful care delivery.
  • Serve as a mentor to caregivers, supporting their care practices.
  • Maintain a composed and positive attitude at all times, demonstrating the ability to multitask in a high-functioning office setting.
  • Exhibit exceptional communication, organizational, and decision-making skills.


The ideal candidate will have:

  • An unencumbered current New York Practical Nurse license.
  • At least two years of experience as an LPN Nurse.
  • The ability to work independently, showcasing sound judgment.
  • Availability for on-call duties outside of standard office hours as required.
  • A valid driver’s license and access to an insured vehicle.
  • Proficiency in learning computer systems.
