Position Overview:

Join our team at 44i as a Practical Nursing Specialist, where you will play a vital role in delivering high-quality nursing services tailored to the unique needs of our clients. We are committed to providing exceptional care through collaboration and professional excellence.

Key Responsibilities:
  • Collaborate with the Interdisciplinary Team to develop, review, and enhance individualized care plans.
  • Assist in conducting comprehensive nursing assessments, including gathering vital signs and relevant health information.
  • Facilitate direct physical examinations and ensure thorough evaluations of clients’ health status.
  • Refer clients to appropriate healthcare providers when necessary.
  • Educate clients and staff on health and hygiene practices, infection control, and symptom recognition.
  • Provide guidance to non-licensed personnel working under medical care plans.
  • Coordinate and schedule essential medical appointments, including physical, dental, and specialist visits.
  • Accompany clients to appointments and outings as required.
  • Administer medications and immunizations in accordance with physician directives, while monitoring for any adverse reactions.
  • Maintain precise medical records and histories for all clients.
  • Deliver basic patient care, including monitoring vital signs and documenting findings.
  • Evaluate the effectiveness of temporary health care plans and collaborate with healthcare team members for improvements.
  • Work alongside the Director of Health Services to develop coordinated care plans based on client assessments.
  • Educate clients on self-administration of medications as appropriate.
  • Provide immediate medical assistance in emergency situations.
  • Participate in an established on-call system to ensure continuous care.
  • Foster a compassionate and supportive environment for clients.
  • Maintain flexibility in scheduling to ensure adequate coverage.
  • Contribute to the efficient operation of the organization by fulfilling assigned duties.
  • Engage in ongoing professional development through continuing education and training opportunities.
  • Uphold client and staff confidentiality in compliance with HIPAA regulations.
Qualifications:
  • Valid Licensed Practical Nurse (LPN) license in the state.
  • General understanding of nursing care methodologies.
  • Strong communication and interpersonal skills, with a focus on teamwork.
  • Excellent organizational abilities and attention to detail.
  • Capacity to follow verbal and written instructions effectively.
  • Preferred experience in developmental disabilities.
  • Genuine respect and commitment to working with individuals with disabilities.
  • Ability to communicate in a culturally competent manner.
  • Successful completion of background checks.
  • Clear tuberculosis screening results.
  • Willingness to complete required training, including CPR/First Aid and other relevant programs.
