Job Description

Aveanna Healthcare is seeking a dedicated and compassionate Direct Support Professional to join our team.

Job Summary:

We are looking for a skilled and empathetic individual to provide high-quality care and support to individuals with intellectual and developmental disabilities (ID/DD). As a Direct Support Professional, you will be responsible for delivering one-on-one services, implementing treatment plans, and promoting independence and self-advocacy.

Key Responsibilities:

  • Implement written goals from treatment plans with consumers, providing personalized support and services.
  • Instruct and reinforce target goals and outcomes, ensuring consumer success and progress.
  • Complete accurate and timely documentation of services provided, as directed by the Quality Professional.
  • Accurately note consumer responses and results to staff interventions and assessments, maintaining a record of progress.
  • Maintain professional conduct at all times, upholding the highest standards of care and respect.
  • Assist in the development of consumer treatment plans and goal revisions, working collaboratively with the interdisciplinary team.
  • Provide care in accordance with written instructions from professional staff, ensuring consumer safety and well-being.
  • Report any changes in the consumer's condition or problems with assigned tasks to the Quality Professional immediately.
  • Relay observations of consumer mental and physical status to the Quality Professional, ensuring timely interventions and support.
  • Be dependable and punctual for assignments, demonstrating a commitment to consumer care and support.

Requirements:

  • Experience working with ID/DD consumers is preferred but not required.
  • Proof of eligibility to work in the United States.
  • High school diploma or GED.
  • Good organizational and communication skills.
  • Active driver's license and current automobile insurance.
