Position Summary:

The Pre-Release Reentry Navigator is an integral member of the Supportive Reentry Network Collaborative (SRNC), a diverse team focused on supporting individuals prior to their release from incarceration. This role involves working within the Cook County Jail to deliver navigation services. The Pre-Release Reentry Navigator will conduct initial assessments and help clients identify essential services to facilitate their transition upon release.

Key Responsibilities:

  • Respond to internal referrals from jail staff promptly.
  • Conduct intake assessments to determine immediate and short-term needs.
  • Utilize Salesforce and other databases for client registration.
  • Facilitate referrals to medical, behavioral health, and housing services to address participants' urgent needs.
  • Collaborate with clients to develop personalized service plans and conduct monthly reviews of progress.
  • Assist clients in overcoming external barriers to achieving their goals as outlined in their service plans.
  • Advocate for clients in various areas, including reentry challenges and risks of homelessness.
  • Assist with Medicaid and SNAP applications, including coverage redeterminations for those recently released.
  • Provide a range of services based on emergent needs, including housing, healthcare, and legal aid.

Outreach and Collaboration:

  • Engage in additional duties as assigned to support the organization's mission and values.
  • Work closely with community partners, hotline responders, and internal staff to support program participants.

Documentation and Compliance:

  • Ensure timely completion of all client documentation.
  • Maintain accurate data entry and conduct weekly self-file reviews.
  • Participate in individual supervision to address client needs and barriers.
  • Adhere to HIPAA and other privacy regulations regarding client information.
  • Attend multi-disciplinary meetings and contribute to continuous quality improvement efforts.
  • Monitor referrals in Salesforce and follow up to ensure compliance.
  • Meet all organizational standards, including productivity metrics.

Qualifications:

  • Ability to represent Safer Foundation professionally and build rapport with diverse clients.
  • Familiarity with reentry populations and best practices.
  • Proficient in computer applications, including Microsoft Office and web-based tools.
  • Experience in case management within behavioral health, corrections, and medical settings.
  • Knowledge of Illinois Medicaid and other public benefits.
  • Understanding of community resources for formerly incarcerated individuals.
  • Ability to troubleshoot and resolve issues effectively.
  • Strong skills in active listening, critical thinking, and conflict resolution.

Education and Experience:

  • Associate degree with three years of case management experience in a social service environment required.
  • Bachelor's degree with two years of relevant experience preferred.
  • Direct lived experience is welcomed.
  • Experience with individuals recently released from incarceration is preferred.
  • Familiarity with community-based resources and public benefits applications.
  • Experience in care coordination and virtual platforms.
