Division Overview: The Corrections Transition Programs (CTP) represent a comprehensive network of clinical reentry management services aimed at enhancing successful reintegration outcomes. CTP offers specialized case management services designed to assist individuals in preparing to reunite with their families and communities following incarceration. Our work spans both correctional facilities and community settings, providing essential reentry support, including behavioral health referrals, public benefits enrollment, employment and housing assistance, and the acquisition of state identification and vital records.

Position Summary: We are seeking a full-time Case Manager to deliver addiction intervention, AODA counseling, and recovery support services to clients and their families. This role is pivotal in uniting services, agencies, resources, and individuals to assist clients in reaching their established objectives. The Case Manager may also engage with high-risk caseloads, co-occurring clients, families, and children.

Key Responsibilities:

  • Facilitate clients in achieving their service plan goals through direct services and coordination with behavioral health, public agencies, and community resources.
  • Assess, evaluate, facilitate, and monitor individual progress in relation to established client-centered service plans and parole conditions.
  • Complete intake documentation for program enrollment and participation.
  • Provide referrals and crisis intervention tailored to the identified needs of individuals.
  • Conduct interviews, review records, and collaborate with other professionals to evaluate individual progress and determine suitability for program participation.
  • Engage with internal and external partners, participate in staff meetings and monthly supervisions, and contribute to integrated staffing with individuals receiving TASC services.
  • Identify and refer individuals with behavioral health needs.
  • Prepare and submit internal and external program reports.
  • Maintain comprehensive documentation related to individuals and programs, including screenings, enrollments, service plans, progress notes, and discharge summaries.
  • Monitor individual progress towards service goals.
  • Assist clients with enrollment in public benefit systems and obtaining vital records.
  • Oversee TASC database systems and external databases to ensure program compliance.
  • Develop effective working relationships with internal and external partners.
  • Participate in meetings to adjust, update, or revise client-centered service plans.
  • Obtain and analyze progress reports from partners to ensure individuals are on track with their service plan benchmarks.
  • Ensure confidentiality of sensitive information related to the organization, its staff, and individuals.

Qualifications:

  • Bachelor's degree from an accredited institution in social work, psychology, criminal justice, or a related field is preferred.
  • Certification and/or licensure as required by contractual obligations.
  • Individuals with criminal records and lived experience are encouraged to apply.
  • CADC/CARS certification may be required within two years of employment based on contractual obligations.
  • Ability to pass IDOC background check.
  • Proficiency in Microsoft Office Suite.
  • Experience in group facilitation.
  • Knowledge of ASAM placement criteria, DSM-V related to substance-related disorders, and Code of Ethics for substance abuse professionals, or the ability to acquire this knowledge.
  • Comfort in correctional settings and with individuals convicted of serious offenses.
  • Commitment to fostering a culture of diversity, equity, and inclusion.

  • Flexibility in thinking and approaches to group discussions.
  • Strong verbal, written, and interpersonal communication skills.
  • Excellent time management skills with a proven ability to meet deadlines.

The agency currently offers hybrid work schedules that combine in-office and remote work. Employees are required to report to the work site three days per week with the option to work up to two days remotely.
