Overview

The Reintegration Care Specialist, under the guidance of the Deputy Director of the Community Resource Center, serves as a case manager for treatment and reentry programs associated with the Cook County Sheriff's Office (CCSO). This position necessitates close collaboration with individuals in custody, community corrections participants, their families, and various community stakeholders, including CCDOC, judicial systems, and local organizations. A significant aspect of this role is partnering with community agencies to facilitate effective service delivery for reentry programs, focusing primarily on housing while also addressing medical, mental health, crisis intervention, substance use, employment, gang disengagement, social security, Medicare/Medicaid, and criminal rehabilitation needs.

Primary Responsibilities

  • Perform comprehensive needs assessments for participants to collect pertinent information for community housing placement.
  • Foster communication and coordination with diverse community housing providers.
  • Create personalized, participant-centered care plans and facilitate community linkage referrals.
  • Assist participants in implementing their care plans and navigating service delivery challenges.
  • Provide precise information and documentation to referral agencies.
  • Advocate for participants and their families throughout the reentry and treatment journey.
  • Collaborate effectively with Electronic Monitoring to secure appropriate housing when necessary.
  • Work alongside CCDOC security personnel and administration as required.
  • Coordinate with all Cook County Government partners and community-based service providers to ensure participants receive essential treatment and services.
  • Operate effectively both as a team member and independently.
  • Manage a significant caseload of participants.
  • Maintain participant records, documentation, data, and referral information meticulously.

Required Knowledge, Skills, and Abilities

  • In-depth understanding of the justice system and the specific needs of justice-involved individuals.
  • Expertise in mental health, substance use, and co-occurring assessment, treatment, and referral processes.
  • Ability to engage effectively with incarcerated individuals and community corrections participants.
  • Supportive and neutral approach towards working with the forensic population and their families.
  • Strong analytical skills for developing appropriate discharge plans.
  • Familiarity with evidence-based practices and their implementation in service delivery.
  • Exceptional attention to detail and thorough record-keeping abilities.
  • Strong communication skills, both verbal and written, with various stakeholders.
  • Proficient in functioning as part of a multidisciplinary team and independently.
  • Commitment to ongoing development of competencies related to the forensic population, cultural diversity, clinical practices, communication skills, personal growth, and ethics.
  • Respect for cultural diversity and ability to work respectfully with individuals from various backgrounds.
  • Understanding and adherence to workplace cultural diversity standards and appropriate conduct.
  • Capability to work remotely if necessary while ensuring participant confidentiality.

Minimum Qualifications

  • Bachelor's degree in social services, psychology, criminal justice, counseling, social work, or related fields.
  • Experience working within CCDOC and with community corrections clients.
  • Proficiency in Microsoft Office Suite.
  • Physical capability to perform duties, including extensive walking within the CCDOC facility.
  • Flexibility to work varied shifts, including evenings, weekends, and holidays.

Preferred Qualifications

  • Master's degree in social work, counseling, psychology, criminal justice, or related field.
  • Progress towards licensure/certification in social work, counseling, or behavioral health.
  • Possession or pursuit of a CADC credential.
  • Minimum of two years' experience within corrections or with justice-involved populations.
  • At least four years of prior experience in behavioral health services and/or social services.
  • Fluency in multiple languages.
