About the Role

CAMBA is a leading provider of social services, dedicated to empowering individuals and families to achieve stability and self-sufficiency. We are seeking a highly skilled and compassionate Client Care Coordinator to join our team at the Park Avenue Family Residence.

Key Responsibilities
  • Assist clients in completing intake applications, screenings, and forms, ensuring accurate and timely documentation.
  • Conduct initial risk assessments of clients and their families, identifying potential areas of concern and developing strategies for support.
  • Prepare and update biopsychosocial evaluations, adhering to regulatory requirements and best practices.
  • Provide instant response services to clients in crisis, ensuring their immediate well-being and safety.
  • Collaborate with clients and case managers to develop and implement Independent Living Plans (ILPs), setting short-term and long-term goals for client success.
  • Identify and connect clients with community resources, medical services, and psychiatric support, facilitating their access to essential services.
  • Follow up with clients and referral organizations to monitor progress and ensure effective service delivery.
  • Consult with internal and external stakeholders to address client challenges and develop solutions.
  • Act as an advocate for clients, arranging for medical, psychiatric, and educational assessments to inform treatment plans.
  • Facilitate group counseling sessions and psychoeducation, promoting social development and psychological education among clients.
  • Monitor client progress, updating service plans and goals as needed to reflect changing circumstances.
  • Document client interactions and progress notes within 48 hours or 3 business days, maintaining accurate and comprehensive records.
Requirements
  • Licensed Master of Social Work (LMSW) required.
  • Must sign NYC DOH/MH attestation, confirming no founded child abuse reports.
