The role of Nursing Program Coordinator at RiverSpring Health involves delivering essential nursing care and support services to residents and their families within a respected long-term care organization. This position is pivotal in ensuring the effective implementation of nursing protocols and is supervised by a Registered Professional Nurse (Director of Nursing).
Key Responsibilities
- Collaborating in the development, execution, and continuous evaluation of individualized nursing care plans.
- Supervising care units and managing Home Health Aides (HHAs) along with their daily responsibilities.
- Monitoring, documenting, and communicating changes in residents' health status, symptoms, and reactions.
- Executing specific nursing tasks within the scope of practice, including the administration of medications and treatments.
- Overseeing resident medication management, including updates, new prescriptions, discontinuations, and refills.
- Facilitating admissions, conducting daily briefings with HHAs, performing audits on new and existing resident charts, and managing incident reporting and follow-ups.
The ideal candidate will be a graduate of an accredited practical nursing program with a valid New York State License and current registration as a licensed practical nurse in accordance with Article 139 of the New York State Education Law. A minimum of one to two years of experience in community nursing, home care, acute care, or adult care settings is required, along with supervisory experience and medication management expertise.
Compensation: $40 per hour
Work Schedule: Monday - Friday with Rotating Weekends
