About the Role

Plymouth Harborside Healthcare is seeking a highly skilled and compassionate Social Worker to join our team. As a key member of our healthcare team, you will play a vital role in promoting the well-being and quality of life for our residents.

Key Responsibilities
  • Conduct thorough assessments of residents' needs and develop personalized care plans to ensure their physical, emotional, and social well-being.
  • Collaborate with residents, families, and healthcare professionals to identify and address any concerns or issues that may impact their care.
  • Provide guidance and support to residents and their families on coping with changes in living arrangements, grief, and loss.
  • Coordinate discharge services, including planning and arranging for follow-up care to ensure a smooth transition for residents.
  • Develop and maintain relationships with community resources and organizations to provide residents with access to additional support services.
Requirements
  • Minimum 1-3 years of experience in a long-term care setting.
  • Bachelor's Degree in Social Work, Psychology, or a related field.
  • Social Work Licensure (LSW, LCSW, or LICSW).
