Key Responsibilities- Build meaningful relationships with residents and their families, providing personalized care and support.
- Develop and implement individualized care plans, working closely with the care team to ensure seamless delivery of services.
- Assist residents with activities of daily living, such as bathing, dressing, and grooming, while promoting independence and dignity.
- Monitor and report changes in resident condition, collaborating with the care team to adjust care plans as needed.
- Participate in the development of Individualized Service Plans (ISPs) and monthly updates, ensuring resident needs are met and care goals are achieved.
- Collaborate with the community team to ensure compliance with OSHA requirements and promote a safe and healthy environment.
- Respond to resident needs and concerns, providing timely and effective support.
- Dedication to providing exceptional care and service to seniors.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, families, and colleagues.
- High School diploma or equivalent required; certifications in senior care or a related field preferred.
- Previous experience working with seniors, preferably in a care management or related role.
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
- Strong problem-solving and decision-making skills, with the ability to think critically and creatively.
