Position Overview

JOIN OUR DEDICATED HOUSING TEAM and contribute to meaningful change in the lives of individuals we serve.

Old Colony Elder Services is on the lookout for two (2) compassionate and committed professionals who thrive in community settings to join our Housing team. This role is essential in providing support and services to residents at our various locations.

Key Responsibilities:

  • Assess the requirements of residents to connect them with appropriate community resources and services.
  • Act as the leader of the Multi-disciplinary Assessment Team, ensuring effective service coordination through regular meetings.
  • Organize care conferences involving residents, their families, and other stakeholders.
  • Design and implement social, educational, and recreational activities for residents.
  • Create and disseminate a newsletter to keep residents updated on upcoming events and activities.

Qualifications:

  • A genuine passion for assisting others and a commitment to making a positive impact.
  • Flexibility to adapt to the evolving needs of residents and programs.
  • A minimum of two years of experience in the human services sector, ideally with the elderly demographic.
  • A Bachelor's degree in Social Work or a related discipline is preferred.
