Job Summary:

We are looking for a Housing Specialist to work with our team to ensure services and support to residents in our Plymouth and Marshfield locations.

Key Responsibilities:

  • Community Outreach: Review the needs of residents to link them to community programs and services.
  • Team Leadership: Serve as the Multi-disciplinary Assessment Team Leader and meet with the team to assure service coordination.
  • Facilitation: Facilitate care conferences with residents, family, and other participants.
  • Program Development: Plan and coordinate social, educational, and recreational programs for residents.
  • Communication: Prepare and distribute a newsletter to keep residents informed of planned activities.

Requirements:

  • Passion for Helping Others: Your passion for helping others and desire to make a difference.
  • Adaptability: Adaptability to be able to change as the needs of consumers and/or programs change.
  • Experience: At least two years of experience in the human services field preferably with the elder population.
  • Education: Bachelor's degree in Social Work or related field preferred.
