About the Role:
The Community Support Specialist for Assertive Community Treatment (ACT) plays a crucial role in delivering an evidence-based, intensive approach to community mental health services. This position is designed to assist at-risk children, youth, and their families in achieving positive changes in their symptoms and behaviors, which can significantly impact their school performance, relationships, living situations, and overall well-being.

Key Responsibilities:
1. Conduct individual assessments to develop tailored care plans that outline presenting issues, objectives, interventions, and expected outcomes.
2. Assist clients in applying for and maintaining adequate health coverage to ensure continuous access to comprehensive healthcare and treatment.
3. Provide outreach, advocacy, coordination, resource identification, and monitoring of both internal and external support services.
4. Deliver counseling services in various settings, including home and school, to help clients develop and reinforce coping strategies that mitigate symptoms and enhance functioning.
5. Facilitate family counseling to educate and empower parents and guardians in supporting their child's recovery and resilience.
6. Offer crisis intervention to ensure client safety during acute symptom exacerbations.
7. Maintain accurate and timely documentation of all services provided.
8. Collaborate with other ACT team members and external providers to ensure comprehensive, wrap-around services align with client needs.
9. Uphold professional standards and confidentiality in all interactions.
10. Participate in relevant committees as directed by supervisors.

Qualifications:
- A Bachelor's degree is preferred; however, an Associate's degree with at least two years of relevant experience may be acceptable.
- Strong clinical judgment in various clinical situations.
- Ability to engage effectively with children in therapeutic settings.
- Proficient in Microsoft Office and electronic health record systems.
- Excellent communication, organizational, and multitasking skills.

Working Conditions:
This role primarily involves working in clients' homes and community settings, with occasional office work and telehealth services. Reliable transportation and a clean driving record are essential.
