Position Summary:

The Community Advocacy Coordinator is essential in promoting community involvement and steering the Community Health Improvement Plan (CHIP) process. This role will establish and facilitate the Community Health Steering Committee, ensuring active participation with a focus on equity across racial demographics. The coordinator's efforts are vital to enhancing health outcomes and addressing social determinants of health in our service areas.

Key Responsibilities:

Community Engagement:

  • Engage with community stakeholders, consumers, and organizations to foster participation in the Community Health Steering Committee and CHIP initiatives.
  • Disseminate information regarding the Community Steering Committee and CHIP through various channels to encourage community involvement.
  • Collaborate with local schools and community organizations to strengthen partnerships.

Steering Committee Leadership:

  • Lead the formation and coordination of the Community Health Steering Committee, involving consumers, staff, and local residents.
  • Facilitate meetings and working groups to collect and analyze data, focusing on health disparities.
  • Guide the committee in establishing priorities and action steps for the CHIP, aligning with community needs.

Data Collection and Analysis:

  • Oversee data collection efforts, including surveys and focus groups, to identify local health priorities.
  • Support the committee in analyzing qualitative and quantitative data to inform strategic development.
  • Ensure data collection emphasizes racial equity indicators.

Partnership Development:

  • Enhance collaborations with organizational partners to coordinate CHIP activities.
  • Encourage partner agencies to join the steering committee for broader community engagement.

Training and Capacity Building:

  • Provide training to community members on facilitation and data analysis.
  • Empower community members with skills to address social determinants of health.

Communication and Reporting:

  • Develop a communication strategy to share CHIP assessment results with stakeholders.
  • Utilize various channels to ensure effective dissemination of information.

Evaluation:

  • Monitor and assess the effectiveness of CHIP activities, adjusting strategies based on feedback.
