Position Summary:

The Community Advocacy Coordinator is integral to NSCH's mission, focusing on enhancing community engagement and leading the Community Health Improvement Plan (CHIP) initiative. Key Responsibilities:

Community Engagement:

  • Collaborate with local educational institutions and community organizations to strengthen partnerships.

Steering Committee Leadership:

  • Lead the formation and coordination of the Community Health Steering Committee, involving consumers, staff, and local residents.
  • Facilitate meetings and working groups to gather and analyze data, focusing on health disparities.
  • Guide the committee in establishing priorities and action plans aligned with community needs.

Data Management:

  • Oversee data collection initiatives, including surveys and focus groups, to identify local health priorities.
  • Assist the committee in analyzing qualitative and quantitative data to inform strategic decisions.
  • Ensure a focus on racial equity indicators in all data-related activities.

Partnership Enhancement:

  • Strengthen collaborations with partner organizations to coordinate CHIP activities effectively.
  • Encourage partner agencies to join the steering committee for broader community engagement.

Training and Development:

  • Provide training to community members on facilitation and data analysis.
  • Empower community participants with skills to address social determinants of health.

Communication Strategy:

  • Develop a communication plan to share CHIP assessment results with stakeholders.
