Role of the Case Manager: As a Case Manager, you will play a vital role in enhancing the well-being and stability of veterans and their families facing homelessness, housing instability, or residing in community-based subsidized housing. Our programs aim to empower individuals to meet their objectives and attain wellness through a Housing First, client-centered, and trauma-informed approach. You will have the opportunity to positively impact the lives of our clients by fostering supportive relationships and connecting veterans to essential resources to secure or maintain permanent housing. Your responsibilities will include developing housing plans and budgets, conducting assessments, making referrals, and coordinating services with VA Medical Centers and other community organizations.

Key Responsibilities:

  • Manage a caseload of veteran clients.
  • Create housing plans and budgets with clients, facilitating connections to resources for maintaining or obtaining permanent housing.
  • Support veteran households in increasing income and preparing them for independent housing maintenance and financial stability through collaboration with our integrated services team.
  • Conduct assessments and coordinate services/referrals with VA Medical Centers and other community organizations.
  • Assist households in applying for housing and subsidy programs, Veterans Administration, Social Security, and other state/federal benefits.
  • Maintain regular communication with veteran households and community partners.
  • Identify and address barriers to permanent housing and conduct housing inspections for potential units.
  • Educate clients on tenant rights and responsibilities.
  • Participate in supervision, team meetings, program audits, training, and professional development.
  • Adhere to professional standards, ensuring all services are documented and client records are maintained.

Qualifications:

  • BA/AA degree preferred; alternatively, a minimum of 2 years of relevant experience in case management may be accepted.
  • Familiarity with veteran issues, homelessness, and state & federal resources is highly desirable.
  • Proficiency in computer applications, particularly electronic health records, Microsoft Word, and Outlook is preferred.
