Veteran Support Case Coordinator

Role Summary: As a Case Manager, you will play a pivotal role in enhancing the well-being and stability of veterans and their families facing homelessness or housing challenges. Our initiatives are designed to empower individuals to achieve their goals and wellness through a Housing First, client-centered, and trauma-informed approach. In collaboration with our veteran service team, you will positively impact the lives of clients by fostering supportive relationships and connecting them to necessary resources for securing permanent housing.
Key Responsibilities:
- Manage a caseload of veteran clients.
- Collaborate with households to create housing plans and budgets, facilitating access to resources for maintaining or securing permanent housing.
- Support veteran households in increasing income and preparing them for independent housing and financial stability through our integrated services.
- Conduct assessments and coordinate referrals with VA Medical Centers and other community organizations.
- Assist households in applying for housing and subsidy programs, as well as other state and federal benefits.
- Maintain ongoing communication with veteran households and community partners.
- Identify and address barriers to permanent housing and conduct inspections for potential housing units.
- Educate clients on tenant rights and responsibilities.
- Participate in supervision, team meetings, audits, training, and professional development opportunities.
- Adhere to professional standards, ensuring thorough documentation and maintenance of client records.
Qualifications:
- BA/AA degree preferred; alternatively, a minimum of 2 years of relevant experience in case management may be accepted.
- Familiarity with veteran issues, homelessness, and state and federal resources is highly desirable.
- Proficiency in electronic health records, Microsoft Word, and Outlook is preferred.
