We are seeking a highly skilled and compassionate Hospice RN Case Manager to join our team at Paradigm Health. As a Hospice RN Case Manager, you will play a vital role in providing high-quality patient care and support to individuals and their families.
Key Responsibilities- Perform comprehensive physical and psychosocial assessments on all hospice patients.
- Develop and implement individualized plans of care to address the unique needs of each patient.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Provide education and support to patients and their families on pain management, symptom control, and end-of-life care.
- Document patient progress and outcomes in a timely and accurate manner.
- One year of Hospice experience preferred.
- Two years of RN experience required.
- Licensed Registered Nurse in the State of Indiana.
- Current driver's license and reliable transportation.
