We are looking for Case Managers who are committed to uplifting Veterans and their families during challenging times.
As a Case Manager, you will play a vital role in enhancing the well-being and stability of Veterans and their families facing homelessness, housing instability, or living in community-based subsidized housing. Our initiatives are designed to empower individuals to achieve their goals and wellness through a Housing First, client-centered, trauma-informed approach. Within our Veteran service team, the Case Manager will significantly impact the lives of clients by fostering supportive relationships and linking Veterans to essential resources for securing or maintaining permanent housing. Responsibilities may include creating housing plans and budgets, conducting assessments, making referrals, and coordinating services with VA Medical Centers and various community organizations.
Key Responsibilities:
- Manage a caseload of Veteran clients.
- Formulate housing plans and budgets with clients, assisting in connecting Veterans to necessary resources for housing stability.
- Support the enhancement of veteran household income and prepare families for independent housing maintenance and financial self-sufficiency in collaboration with our integrated services team.
- Conduct assessments and facilitate service coordination/referrals with VA Medical Centers and other community organizations.
- Assist households in applying for housing and subsidy programs, Veterans Administration benefits, Social Security, and other state/federal assistance.
- Maintain consistent communication with Veteran households and community partners.
- Identify and address barriers to permanent housing and conduct housing inspections for potential units.
- Educate clients on fundamental tenant rights and responsibilities.
- Engage in individual supervision, team meetings, program audits, training sessions, and other professional development opportunities.
- Adhere to professional standards, ensuring all services are documented and client records are maintained accurately.
Qualifications:
- BA/AA degree preferred; however, a minimum of 2 years of relevant experience in case management may be considered.
- Familiarity with veteran issues, homelessness, and state & federal resources is highly desirable.
- Proficiency in computer applications, particularly electronic health records, Microsoft Word, and Outlook is preferred.
