The HUD-VASH Program represents a collaborative initiative aimed at assisting Veterans and their families in overcoming homelessness by facilitating their transition into stable housing. This program integrates long-term case management with supportive services to ensure comprehensive care for chronically homeless Veterans.

Position Summary:

The Senior Case Manager III serves as the primary expert within the case management team. This role involves mentoring team members and, under the guidance of the Program Coordinator, ensuring that all services provided by the case management team effectively address client needs. Responsibilities include:

  • Conducting thorough assessments to gather essential information regarding clients' functional, environmental, financial, and health situations to create tailored case plans.
  • Establishing support systems that cater to client requirements, including coordinating move-in logistics and service referrals.
  • Regularly monitoring case managers to evaluate the quality and impact of services delivered.
  • Facilitating access to a variety of community resources and services to support clients in achieving self-sufficiency.
  • Performing comprehensive reviews of case files to ensure all necessary documentation is accurately maintained.
  • Leading workshops that align with client needs and participating in outreach initiatives.

Qualifications:

Requirements:

  • Successful completion of a background check and fingerprint clearance.
  • Current TB screening and immunization records.
  • Valid driver's license and professional license (if applicable).
  • Completion of all mandatory training within the specified timeframe.

Education:

  • Bachelor's degree in social sciences or family therapy from an accredited institution.
  • Completion of specialized case management training within six months of hire.

Experience:

  • A minimum of five years of case management experience, particularly with high-need populations.
  • One year of experience may be substituted with a master's degree in social sciences.

Preferred Qualifications:

  • Master's degree in Social Work or a related field.
  • Experience in a supervisory or leadership capacity.
  • Familiarity with HMIS navigation.
