Position Summary:

The Senior Case Manager III serves as the key expert within the case management team. This role involves mentoring fellow team members and, under the guidance of the Program Coordinator, ensuring that all services provided by the case management team are tailored to meet client needs effectively. Responsibilities include assisting case managers in evaluating client requirements, educating them about available resources, developing actionable service plans, scheduling necessary appointments, and ensuring that follow-ups are conducted to monitor progress.

Key Responsibilities:

  • Conduct thorough assessments with clients to gather comprehensive information regarding their functional, environmental, financial, and health needs to formulate a personalized case plan.
  • Establish support systems to address client needs, including coordinating move-in activities and service referrals.
  • Regularly monitor case managers to evaluate the quality and effectiveness of services delivered.
  • Assist case managers in identifying and coordinating necessary services to help clients achieve self-sufficiency and stability.
  • Conduct file reviews to ensure all documentation is complete and accurate for program eligibility.
  • Facilitate workshops and outreach events that align with client needs.
  • Perform additional duties as assigned.

Qualifications:

  • Ability to pass background checks, including criminal history and employment verification.
  • Current TB screening and immunization records.
  • Possession of a valid driver's license.
  • Completion of all required training within specified timeframes.

Education:

  • Bachelor's degree in social sciences or family therapy from an accredited institution.
  • Completion of specific case management training within six months of employment.

Experience:

  • A minimum of five years of case management experience with high-need populations.
  • One year of experience may be sufficient for candidates with a master's degree in social sciences.

Preferred Qualifications:

  • Master's degree in Social Work or a related field.
  • Experience in a supervisory or leadership role.
  • Familiarity with HMIS navigation.
