Program Insight:

The HUD-VASH initiative represents a collaborative effort among Volunteers of America, the Department of Housing and Urban Development, and the Department of Veterans Affairs. This program aims to transition veterans and their families from homelessness to stable, permanent housing. It offers comprehensive case management, clinical support, and housing assistance tailored for chronically homeless veterans.

Position Summary:

The Senior Case Manager III serves as the primary expert within the case management team. This role involves mentoring fellow team members and, under the guidance of the Program Coordinator, ensuring that all services provided align with client needs. Responsibilities include assisting case managers in evaluating client requirements, informing them about available resources, crafting personalized service plans, scheduling necessary appointments, and conducting follow-ups to ensure the effectiveness of the service plans.

Key Responsibilities:

  • Perform thorough screenings and assessments to gather vital information regarding clients' functional, environmental, financial, employment, housing, educational, and health circumstances to create effective case plans.
  • Establish support networks to address client needs, facilitate move-in processes, negotiate rental agreements, and provide service referrals as necessary.
  • Regularly monitor case managers to evaluate the quality and impact of the services rendered.
  • Assist case managers in identifying and coordinating a variety of services essential for veterans and families to achieve self-sufficiency and stability.
  • Conduct comprehensive reviews of case files to ensure all required documentation is collected for eligibility in program services.
  • Develop and facilitate workshops that address the needs of clients.
  • Engage in outreach initiatives to promote program services.
  • Perform additional duties as assigned.

Qualifications:

  • Ability to pass a comprehensive background check, including criminal history and employment verification.
  • Current TB screening and immunization records.
  • Possession of a valid driver’s license.
  • Completion of all mandatory training sessions.

Educational Background:

  • Bachelor's degree in social sciences or family therapy from an accredited institution.
  • Completion of Volunteers of America Case Management training within the first six months of employment.

Experience Requirements:

  • A minimum of five years of proven case management experience with high-need populations.
  • For candidates with a master's degree in social sciences, only one year of case management experience is required.

Preferred Qualifications:

  • Master's degree in Social Work or a related field.
  • Experience in a supervisory or leadership capacity is preferred.
  • Familiarity with HMIS navigation.
