Job Title: Outreach and Engagement Case Manager

Special Service for Groups, Inc- HOPICS is seeking a highly skilled and compassionate Outreach and Engagement Case Manager to join our team. As a key member of our staff, you will play a vital role in providing trauma-informed care and support services to individuals and families in need.

Job Summary:

The Outreach and Engagement Case Manager will serve as the primary representative for our organization during outreach and engagement events, providing professional system navigation, service coordination, and advocacy services to individuals and families affected by trauma and mental health issues. This role requires a strong understanding of trauma-informed care principles, excellent communication and interpersonal skills, and the ability to work effectively in a fast-paced environment.

Key Responsibilities:
  • Serve as the primary representative for our organization during outreach and engagement events, providing information and support to individuals and families in need.
  • Provide professional system navigation, service coordination, and advocacy services to individuals and families affected by trauma and mental health issues.
  • Work collaboratively with community partners, law enforcement, and other stakeholders to provide comprehensive support services.
  • Develop and maintain relationships with community-based organizations, service providers, and other stakeholders to ensure seamless service delivery.
  • Provide technical assistance and support to clients, including accompaniment to appointments and court appearances.
  • Develop and maintain accurate and comprehensive records of client interactions, services provided, and outcomes.
  • Participate in regular team meetings and training sessions to stay up-to-date on best practices and program developments.
Requirements:
  • Bachelor's degree in Social Work or a related field, or equivalent experience.
  • Minimum of 2 years of experience working in a trauma-informed care setting, preferably in a community-based organization.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Strong understanding of trauma-informed care principles and practices.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and partnership-building.
  • Valid California driver's license and reliable transportation.
