Key Responsibilities:


Conduct comprehensive screenings and assessments with behavioral health recipients to identify their presenting issues, determine their needs, and formulate suitable service plan objectives.

Develop and implement initial individualized service plans.

Prepare intake documentation, progress notes, event notifications, and other necessary records regarding behavioral health recipients in compliance with regulatory standards and organizational policies.

Ensure timely completion of required documentation, including screenings, assessments, and service plans.

Facilitate care coordination, which includes notifying program staff of new behavioral health recipient intakes, summarizing intake information, and recommending appropriate services/programs.

Gather essential information from new members through courteous and professional surveys or interviews.
Assist in collecting data to support the admission process within health or mental care facilities.
Ensure all admission forms are accurately signed and filed.
Uphold the dignity and confidentiality of members or patients.
Participate in clinical staff meetings, training sessions, and other required activities.

Must be capable of working independently across multiple sites while addressing varied member needs, as well as functioning as a vital part of a trans-disciplinary team.

Availability for on-call duties after hours as assigned; required to carry a company phone and document hours of work and location.
May be required to operate a SBH vehicle for consumer transportation, as determined by clinic and/or program discretion.

Education & Qualifications:
The minimum requirement is to be at least 21 years of age.
A bachelor's degree (in a related or unrelated field) in behavioral health, along with one year of relevant experience, is required. A combination of experience and education totaling five years is acceptable.
Must maintain personal auto insurance in accordance with SBH guidelines.

Fingerprint clearance issued by the Arizona Department of Public Safety is required or must be obtained within eight (8) weeks of hire.
