The Transplantation Nursing Coordinator is responsible for orchestrating, directing, and delivering specialized nursing care focused on individuals facing end-stage organ disease who are slated for solid organ transplantation. This role emphasizes continuity and coordination of patient care, alongside conducting educational initiatives for both patients and professionals, while fostering collaborative relationships with multidisciplinary teams and community resources.
Key Responsibilities and Essential Functions
- Exhibit expertise in facilitating living donation and transplantation, coordinating the care of living donors for evaluation and organ donation.
- Oversee the care of transplant recipients in outpatient settings, ensuring optimal physical, social, and emotional rehabilitation while addressing the educational and psychosocial needs of recipients and their families.
- Reinforce post-operative education and ensure comprehensive data collection regarding recipients' health and organ function.
- Communicate critical lab data and diagnostic results to the transplant physician and implement necessary changes in treatment as directed.
- Ensure annual malignancy screenings and assess the need for immunizations for recipients.
- Monitor and coordinate care for candidates awaiting organ transplantation, developing tailored educational plans to meet their needs.
- Maintain effective communication with patients, referring physicians, payers, and transplant team members.
- Conduct regular cytotoxic antibody screenings as per transplant center protocols and maintain health surveillance during the waiting period.
- Demonstrate proficiency in managing candidate-listing status in compliance with OPTN/UNOS regulations.
- Facilitate candidate evaluations and coordinate multidisciplinary team efforts throughout the evaluation process.
- Manage post-transplant care, including medication management, complication identification, and discharge planning.
- Educate candidates and their families about living donation options, the evaluation process, and post-operative responsibilities.
- Act as a liaison between donor and recipient transplant teams, ensuring seamless communication and coordination.
- Maintain thorough documentation of all processes and patient interactions, ensuring compliance with regulatory standards.
- Engage in continuous professional development by reviewing current literature, attending conferences, and participating in transplant-related organizations.
- Mentor and guide newly hired staff, sharing knowledge and best practices in transplantation nursing.
- Demonstrate accountability for delivering high-quality care to transplant candidates, recipients, and living donors.
- Ensure adherence to OPTN/UNOS policies and effectively communicate transplant listing decisions to patients and referring physicians.
- Evaluate potential transplant candidates for eligibility, considering co-morbidities and contraindications.
- Provide comprehensive education on immunization requirements prior to transplantation.
- Must possess the clinical competencies required for the assigned unit or department.

- High School Diploma
- Associate Degree in Nursing; Bachelor's Degree in Nursing must be completed within five years of hire/transfer.
- Minimum of 2 years of acute clinical experience.
- Bachelor's Degree in Nursing.
- Basic Life Support Heartsaver (BLS HS) certification from the American Heart Association (AHA) within 14 days of hire.
- Licensed Registered Nurse (LRN) in the state of Kansas.
- Must hold a current active multistate RN license in good standing or an active single state RN license with proof of application for a multistate RN license.
