Position Overview:
The Community Outreach Representative plays a pivotal role in connecting with Medicare seniors to stimulate organic patient growth within local communities. This position collaborates closely with community partners and healthcare facilities to ensure seniors have access to preventive primary care that is value-based. The role will report to the Outreach Community Manager.

  • Key Responsibilities:
  • Facilitate and deliver educational presentations to both current and prospective patients.
  • Establish and nurture professional relationships with referral sources.
  • Participate in community events and activities aimed at generating leads.
  • Engage potential members through various communication channels including phone, text, and email.
  • Track and manage leads utilizing CRM systems or similar tools.
  • Collaborate with health plan partners to enhance service delivery.
  • Build and maintain relationships with local brokers.
  • Work with marketing teams to promote community events.
  • Conduct tours for prospective patients in collaboration with operational staff.
  • Forge partnerships with local non-profits to attract new patients.
  • Coordinate with clinic staff to organize services and events for potential patients.
  • Engage with brokers and local health plans to boost awareness and enrollment.
  • Collaborate with marketing for social media engagement and content creation.
  • This position is hybrid, involving both fieldwork and home office responsibilities (50% each).
  • Convert prospective patients into enrolled and active patients with scheduled visits and assigned primary care providers.
Qualifications:
  • Bachelor's degree in business, marketing, sales, or equivalent experience.
  • A minimum of two years of experience in the healthcare industry.
  • At least one year of experience with Medicare; familiarity with Value-Based Care is advantageous.
  • Bilingual proficiency is required.
  • Experience in a quota-driven compensation model.
  • Proficient in PowerPoint and possess strong presentation skills.
  • Skilled in reporting and tracking data using Excel.
  • Exceptional written and verbal communication abilities.
