Position Overview:
The Community Outreach Representative plays a crucial role in connecting with Medicare seniors to facilitate organic patient growth within local communities. This position collaborates closely with community partners and healthcare facilities to ensure seniors have access to essential preventive primary care services. Reporting to the Outreach Community Manager, this role encompasses a variety of responsibilities aimed at enhancing community health engagement.
- Conduct and coordinate informative presentations for both current and prospective patients.
- Establish and nurture relationships with referral sources to enhance patient outreach.
- Participate in community events and clinic activities to generate leads.
- Engage potential members through various communication channels including phone, text, and email.
- Track and manage leads using CRM systems or other tools.
- Collaborate with health plan partners to streamline coordination efforts.
- Build and maintain relationships with local brokers.
- Work alongside marketing teams to promote community events effectively.
- Facilitate tours for prospective patients in collaboration with operational staff.
- Develop partnerships with local organizations, including non-profits, to attract new patients.
- Coordinate with clinic staff to organize services and events for potential patients.
- Engage with brokers and local health plans to raise awareness and boost patient enrollment.
- Collaborate with marketing for social media engagement and content creation.
- This position involves a hybrid work model, balancing fieldwork and home office responsibilities.
- Convert prospective patients into enrolled members with scheduled visits and assigned primary care providers.
- Bachelor's degree in business, marketing, sales, or related field, or equivalent professional experience.
- A minimum of two years of experience in the healthcare industry.
- At least one year of experience with Medicare or Value-Based Care is advantageous.
- Bilingual proficiency is required.
- Experience in a quota-driven compensation model is preferred.
- Proficient in PowerPoint and possess strong presentation skills.
- Experience in reporting and tracking data using Excel.
- Exceptional written and verbal communication abilities.
