About the Role

At Healthcare in Action, we are seeking a highly skilled and compassionate Care Management Supervisor to join our team. As a key member of our organization, you will play a vital role in enhancing the health and well-being of individuals experiencing homelessness.

Key Responsibilities
  • Develop and Implement Care Plans: Utilize evidence-based guidelines and clinical knowledge to create individualized care plans for clients, conducting onsite visits, gathering assessment information, and identifying problems, goals, and interventions.
  • Deliver Ongoing Care Coordination: Coach, educate, navigate, and refer clients to appropriate care and community-based resources, promoting self-management and independence.
  • Supervise and Lead the Care Management Team: Provide guidance, support, and oversight to the Care Management team, ensuring effective coordination of patient care and successful outcomes.
  • Develop Cross-Functional Relationships: Establish and maintain strong relationships with housing and allied community agencies, attending meetings, working on-site, and structuring effective communication mechanisms.
  • Document and Record: Complete timely and accurate documentation in multiple computer systems, recording assessment and corresponding documentation, including care plans and progress notes.
Requirements
  • Education: Bachelor's or Master's Degree in Social Work, Gerontology, or a related field of study.
  • Experience: 2-3 years of experience in homeless care management, with a Bachelor's Degree, or 2+ years of experience with a Master's Degree.
  • Skills: Strong interpersonal, critical thinking, and analytical skills, with excellent written and verbal communication skills, organizational skills, and ability to multitask.
