About the Role

We are seeking a highly skilled and compassionate Care Management Supervisor to join our team at Healthcare in Action. As a key member of our care management team, you will play a critical role in enhancing the health and well-being of individuals experiencing homelessness.

Key Responsibilities
  • Develop and Implement Care Plans: Utilize evidence-based guidelines and clinical knowledge to create individualized care plans for clients, conducting onsite visits, gathering assessment information, and identifying problems, goals, and interventions.
  • Provide Ongoing Care Coordination: Deliver care coordination and care management by coaching, educating, navigating, and referring clients to appropriate care and community-based resources.
  • Support Client Self-Management: Assist patients in taking an active role in managing their health and social needs by coaching, using motivational interviewing, and educating about self-management tools and strategies.
  • Build Cross-Functional Relationships: Develop strong relationships with housing and allied community agencies by attending meetings, working on-site, and establishing effective communication mechanisms.
  • Lead Care Management Team Efforts: Coordinate with patients' various care teams to identify barriers to care plans by sharing assessment information.
  • Document and Record: Complete timely and accurate documentation in multiple computer systems to record assessment and corresponding documentation, including care plans and progress notes.
  • Exhibit Strong Interpersonal and Analytical Skills: Demonstrate excellent interpersonal, critical thinking, and analytical skills through positive communication with clients, caregivers, care teams, and community agencies.
  • Enhance Skills and Knowledge: Participate in team case conferences and training, per department guidelines, to enhance skills and knowledge.
  • Maintain Confidentiality and Adhere to Standards: Maintain the client's right to privacy and protect HIA operations by keeping information confidential and ensuring Case Management does the same, adhering to all quality, compliance, and regulatory standards to achieve outcomes.
Qualifications and Experience
  • Education and Experience: Bachelor's or Master's Degree in Social Work, Gerontology, or related field of study preferred, but not required. A comparable combination of education/experience and/or training will be considered equivalent to the education listed above.
  • Leadership Experience: 0-1 year of people leadership with some direct supervision experience.
  • Homeless Care Management Experience: 2-3 years of experience in homeless care management with a Bachelor's Degree, or 2+ years of experience with a Master's Degree.
  • Desirable Skills and Knowledge: Demonstrated knowledge of assessment, health, and functional problems of homeless older adults, knowledge of community resources for seniors, basic knowledge of related CMS and DHCS regulations, strong interpersonal skills, including excellent written and verbal communication skills, strong organizational skills, ability to multitask, ability to maintain confidentiality, and strong analytical and critical thinking skills.
