Position OverviewThe Clinical Care Progression Supervisor is a highly skilled clinician responsible for overseeing care progression and coordination across a designated area, typically involving multiple patient care units.
Reporting to the Director of Care Progression and Transitions Management, this supervisory role entails managing a team of unit-based Care Managers and Transition Coordinators.
The Supervisor ensures that care is provided in the appropriate setting, at the right time, and for the necessary duration, with a strong focus on facilitating safe and smooth transitions to subsequent levels of care.
Additionally, the Supervisor leads daily care briefings and fosters collaboration among interprofessional team members to enhance care progression.
This individual also acts as a coach, mentor, and role model for bedside RNs and other interprofessional staff members on individual units.
Moreover, the Supervisor identifies obstacles to care progression and coordination, escalates issues to relevant parties, and follows through to ensure resolution.
The incumbent serves as the clinical subject matter expert, overseeing assessment, planning, implementation, and evaluation of all patient-centered care coordination activities.
The Supervisor is tasked with developing, enforcing, and auditing standard work processes, as well as conducting performance evaluations for their assigned team.
Cross-training to step in as a care manager or transitions coordinator is also expected when necessary, as determined by the Care Coordination Leadership team.
Success in this role is rooted in the ability to demonstrate effective, professional communication with patients, families, physicians, and both departmental and interdepartmental staff to optimize workflow and facilitate problem identification and resolution.
The employee supports MemorialCare's strategic objectives and actively participates in Lean initiatives aimed at continuous performance improvement, patient safety, and patient-centered care.

Key ResponsibilitiesLeadership and Supervision:
Directly supervises the care coordination team by establishing expectations and managing accountability systems. Conducts staff performance evaluations and assists in training new personnel. Provides guidance to team members regarding performance improvement opportunities. Acts as a coach and mentor for staff, nurses, and interprofessional team members.
Continuum of Care:
Serves as the clinical subject matter expert for care progression activities, including identifying and addressing barriers. Oversees care coordination and transition planning activities, ensuring continuity of care and effective handovers between providers of care and services.
Collaboration and Partnership:
Works closely with Nursing Operations Leadership to ensure high-quality patient care and nursing excellence. Collaborates with Patient Throughput Leadership to facilitate seamless and timely patient flow for admissions, transfers, and discharges.
Advocacy and Empowerment:
Advocates for appropriate care delivery, ensuring it occurs at the right time and in the right setting. Facilitates coordination among the interprofessional team, services, and settings involved in patient care, prioritizing the rights and safety of patients and families throughout the care continuum. Serves as a resource to assist staff in overcoming barriers to safe patient discharge. Empowers patients to maintain control over their treatment and self-management.
Physician Liaison:
Supports team members and interprofessional staff by acting as a liaison with physicians/providers to ensure the patient's care plan is progressing effectively. Escalates barriers to the appropriate Medical Director or Physician Advisor for resolution.
Quality Improvement:
Embraces a Lean mindset and utilizes the Plan-Do-Study-Act (PDSA) performance improvement model. Actively identifies opportunities for improvement, supports data collection, and leads quality improvement initiatives.
Information Management:
Utilizes software systems, websites, electronic devices, and other information systems in accordance with standard work and policies.
Staff Engagement:
Promotes employee engagement, inspiring high-performing team members and leading by example. Cultivates a supportive and healthy work environment through clinical and administrative leadership and expertise. Fosters an environment that enhances service excellence and the patient and family experience.
Minimum Qualifications
Experience:
5 years of experience as a bedside nurse is required. Experience as a charge nurse, team lead, or in care coordination is strongly preferred. A demonstrated aptitude or desire for supervising a small team is essential. 7 to 10 years of case management experience, with a minimum of 2 years in an acute inpatient setting, is preferred. Experience managing culturally diverse patient populations is also important.
Education/Certification:
RN License in California is required. A BSN is mandatory, while an MSN is preferred; a Master's degree in a related health field may be considered. Current BLS for Healthcare Providers certification is required. ACM or CCM certification is preferred.
