The Patient Care Coordinator plays a vital role in assisting families to navigate and access community resources while providing support through the management of population health initiatives and care coordination efforts. This position collaborates to arrange necessary services for patients and fosters relationships with local organizations, educational institutions, faith-based groups, and various community programs. Patient Care Coordinators engage directly with patients and families during inpatient admissions, outpatient specialty clinic visits, emergency room interactions, home visits, community outreach, and through telephonic communication.

Key Responsibilities


• Engage and inspire members to participate in various case management initiatives by clearly communicating objectives, advantages, and interventions. Assist in developing family-centered care plans.
• Gather information for customized assessments related to case management eligibility and refer as necessary when criteria for licensed clinical intervention are met. Facilitate communication and collaboration among the healthcare team.
• Provide personalized navigation support to members, helping them maneuver through the healthcare system. Connect families with resources to address social determinants of health while accommodating cultural and linguistic needs. Oversee transitions between settings, caregivers, and providers, ensuring follow-up throughout the continuum of care.
• Conduct outreach to primary care providers, specialists, and home care services to research and facilitate service referrals. Enhance patients' and families' self-management skills through education and resource provision.
• Handle incoming calls, schedule appointments, and assist members in resolving immediate needs in real-time. Manage administrative tasks to support the program.
• Prepare and maintain records and case files, including documentation of clients' personal and eligibility information, services rendered, progress towards goals, and significant changes.
• Participate in the orientation and ongoing education of staff and students as appropriate.

Qualifications

Education: Bachelor's degree in healthcare, public health, or a related clinical field is preferred.
Licensure: Valid driver's license and proof of auto insurance as required by hospital policy. Must pass a motor vehicle background check and meet insurance eligibility and driving qualifications set by Nationwide Children's Hospital.
Certifications: Active Basic Life Support (BLS) certification is required.
Skills: Proficient understanding of Medicaid and other regulatory agency standards is required.
Experience: Minimum of 2 years of experience in a patient-facing role within the healthcare sector is required.
