We are seeking a Patient Care Coordinator to play a vital role at AndHealth. In this position, you will assist patients throughout their healthcare journey at our partner locations in Columbus, OH, aiming to achieve outstanding health outcomes and ensuring they receive the necessary care for a healthier lifestyle.

You will collaborate with our Physicians, Nurses, Health Coaches, and Patient Services team, providing insights on how to enhance the patient experience.

Key Responsibilities:
  • Prepare examination rooms for patient consultations by ensuring they are well-stocked and sanitized.
  • Manage patient communications, receive and direct test results for provider evaluation, and gather information regarding patient medications.
  • Assist in rooming patients, checking vital signs, and assessing patient needs to relay to the provider.
  • Support the provider during patient procedures as directed.
  • Communicate effectively, both verbally and in writing, with patients, their families, colleagues, and external parties.
  • Complete necessary documentation for patients as required.
  • Ensure timely and accurate charting in the Electronic Health Record (EHR).
  • Conduct laboratory tests as instructed by the provider, prepare specimens, operate lab equipment, and maintain laboratory records in compliance with CLIA standards and specific protocols.
  • Reconcile billing from external laboratories by comparing daily lab logs with patient EHR.
  • Coordinate scheduling for offsite diagnostic tests and communicate effectively with patients.
  • Facilitate the referral process for patients as necessary.
  • Maintain manual inventory of medications.
  • Perform front office tasks as required.
  • Adhere to all policies and procedures of the community health center.
  • Other duties as assigned based on organizational needs.
Education & Licensure Requirements:
  • Bachelor's degree is preferred.
  • LPN certification is required.
  • RN certification is preferred.
  • American Heart Association Basic Life Support Provider (BLS) certification required or willingness to obtain.
Additional Skills and Qualifications:
  • Experience in an outpatient or ambulatory care setting is preferred.
  • Proficiency in Microsoft Office applications.
  • Ability to navigate Electronic Medical and Health Record (EMR, EHR) systems.
  • A strong sense of ownership and initiative to enhance each patient's experience.
  • Excellent verbal and written communication skills.
  • Attention to detail and ability to manage multiple tasks simultaneously.
  • A servant-leadership approach in interactions with all individuals.
  • Comfortable with technology and eager to learn new systems.
  • Enjoy working collaboratively with diverse teams across various organizations.
Additional Requirements:
  • Proof of negative TB skin test within the last 12 months.
  • Documentation of immunity to Measles, Mumps, and Rubella or a blood titer.
  • Documentation of two doses of Varicella or a blood titer proving immunity.
  • Completion of Hepatitis B series.
  • Influenza vaccination required.
  • COVID vaccination required.
  • Additional health requirements may be necessary.
