Job Summary

We are seeking a highly motivated and detail-oriented Patient Navigator to join our team at the Ohio State University Wexner Medical Center. The Patient Navigator will play a critical role in promoting health and well-being among patients in our Emergency Departments Health Now Program.

Key Responsibilities:
  • Screen patients and receive referrals for potential eligibility in Health Now Program activities
  • Approach patients and explain Health Now Program processes and services
  • Consent and enroll participants in eligible research studies
  • Complete system-approved point-of-care clinical and confirmatory tests for diagnostic screening and assessment
  • Refer patients to the next level of care or linkage services as indicated
Essential Functions:
  • Address patient concerns through active listening and escalate as needed to the care team
  • Research and remain up-to-date on available services and programs within and outside of the OSU network to successfully match patient needs with resources
  • Maintain appropriate tracking and reporting systems and complete other related duties as assigned
  • Work closely with clinical services and linkage to care teams to ensure referrals are completed and patients' needs are assessed
Research Role:
  • Provide assistance in support of clinical research studies being conducted in the Department of Emergency Medicine
  • Participate in the recruitment of study participants
  • Collect and provide relevant data to be used in assessing eligibility of patients for participation in protocols
  • Assist with administration of informed consent process
  • Assist with cognitive testing both clinically and for research visits
  • Assist principal investigator and sub-investigator with continuity of care of study patients during and between clinic visits
  • Assist with collection, entering, maintenance, and reporting of clinical research data
  • Assist with analysis and interpretation of research data
Requirements:
  • Bachelor's degree in a related field
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment
  • Strong analytical and problem-solving skills
