Position Overview:
The Nurse Care Coordinator (NCC) plays a pivotal role in orchestrating patient and family-centered care activities among multiple stakeholders. Utilizing advanced critical thinking skills, the NCC is tasked with the development, implementation, and evaluation of personalized care plans aimed at empowering patients and enhancing health outcomes.

Key Responsibilities:
The NCC is responsible for marshaling and coordinating resources and patient care activities to facilitate effective transition management. This role involves:
  • Demonstrating the application of professional nursing standards and ethical practices in all aspects of care.
  • Planning, coordinating, and prioritizing patient care activities while considering individual patient needs and desired outcomes.
  • Ensuring safety and continuity of care through effective documentation and communication processes.
  • Collaborating with patients, families, and caregivers to support informed healthcare decisions that respect cultural values.
  • Providing tailored health education and coaching based on identified treatment needs.
  • Exhibiting leadership that fosters health equity and enhances population health.
  • Engaging in research and applying evidence-based practices to improve patient care.
  • Leading efforts to maintain continuity of care across various services.
  • Educating patients with chronic conditions on self-management strategies.
  • Identifying support needs and developing actionable care plans.
  • Facilitating communication and collaboration to evaluate treatment outcomes.
  • Supporting medication management as required.
  • Performing additional duties as assigned.

Physical Requirements:
