Healthcare Nurse Coordinator (HNC)

The HNC plays a pivotal role in orchestrating patient and family-focused care initiatives, crafting personalized care strategies, and managing resources to facilitate seamless transitions in care. This position demands advanced analytical skills and collaboration across various healthcare sectors.

Core Responsibilities:

  • Executing the Professional Practice Framework
  • Collaborating on patient care tasks with the multidisciplinary team
  • Ensuring safety and continuity in patient care
  • Delivering health education and guidance to patients
  • Advocating for health equity and supporting care transitions
  • Instructing patients with chronic conditions on self-management techniques
  • Engaging in research and applying evidence-based methodologies
  • Guaranteeing consistent and cohesive care throughout the healthcare continuum

Compensation Range: $85,000 - $113,401.60 Annually

Essential Qualifications:

  • Education: Bachelor of Science in Nursing
  • Experience: Minimum of 1 year in registered nursing required
  • Specialty Board Certification is highly desirable
  • License/Certification: Registered Nurse in the Commonwealth of Virginia

Physical Requirements:

This role necessitates prolonged periods of standing, frequent travel, and meticulous attention to detail. Candidates must possess effective communication, auditory, and visual capabilities.
