Position Overview:
The Nurse Navigator collaborates with a diverse healthcare team to orchestrate and enhance the delivery of care for a specific patient demographic within the Medical Center. This role is pivotal in providing education and support to patients and their families, ensuring seamless transitions of care, and facilitating appropriate referrals and follow-up appointments after discharge.

Key Responsibilities:
  • Work in conjunction with care management to assess and monitor patients upon admission for potential needs, including medication assistance and access to community resources.
  • Design personalized care plans in partnership with the multidisciplinary team.
  • Collect essential information to inform the development of self-management strategies for patients, addressing psychological, social, and economic factors.
  • Identify and coordinate care for specific disease populations among inpatients.
  • Deliver, reinforce, and document education related to disease management and health maintenance for patients and/or caregivers.
  • Recognize potential obstacles to effective disease management and reasons for readmissions.
  • Collaborate with established multidisciplinary teams to devise suitable interventions to overcome identified barriers.
  • Implement strategies to address both short-term and long-term care requirements.
  • Manage and coordinate care before and after discharge, ensuring continuity and quality.
  • Assist in evaluating patient care outcomes to enhance service delivery.
  • Exhibit leadership in patient care at the Clinician 3 level.
  • Coordinate patient education and care initiatives.
  • Participate in performance measurement and improvement efforts.
  • Additional responsibilities may be assigned as needed.

Compensation:
Position Compensation Range: $54.52 Hourly

Minimum Qualifications:
  • Education: Graduate of an accredited nursing program; Bachelor's of Science in Nursing Degree preferred.
  • Experience: Minimum of 1 year of relevant experience.
  • Licensure: Must be licensed to practice as a Registered Nurse in the Commonwealth of Virginia. AHA Health Care Provider BLS certification is required. A valid Virginia Driver's License may be necessary.

Physical Requirements:
This position involves prolonged standing, frequent travel, and bending/stooping. Candidates must possess proficient communication, auditory, and visual skills, with a strong attention to detail and the ability to write legibly. The role may require lifting, pushing, or pulling heavy objects and exposure to chemicals, blood/body fluids, and infectious diseases.
