As a Clinical Nurse Care Coordinator, you will play a vital role in evaluating and addressing the needs of patients and their families to ensure a smooth and efficient experience within the healthcare system. Your primary responsibilities will include conducting initial evaluations for all patients from the onset of their admission to facilitate a seamless discharge process. You will be tasked with performing level of care evaluations, guaranteeing that patients receive the appropriate levels of care, and ensuring smooth transitions throughout their healthcare journey.

Compensation Range: $67,000 - $108,555.20 Annually

MINIMUM REQUIREMENTS

Education: Must be a graduate of an accredited nursing program, with a Bachelor of Science in Nursing required within five years of employment.

Experience: A minimum of three years of relevant experience is required.

Licensure: Must hold a valid Registered Nurse license in the Commonwealth of Virginia. Accreditation as a Case Manager (ACM) is required within three years of employment.

PHYSICAL DEMANDS

This position requires prolonged periods of standing, frequent travel, and the ability to bend or stoop. Proficient communication, auditory, and visual skills are essential; attention to detail and the ability to write legibly are necessary. The role also requires the ability to lift, push, or pull weight as needed. Exposure to chemicals, blood/body fluids, and infectious diseases may occur.
