Job Summary

The University of Virginia is seeking a highly skilled Registered Nurse Care Coordinator to join our team. As a key member of our healthcare team, the successful candidate will be responsible for coordinating patient care activities and developing individualized care plans to optimize patient outcomes.

Key Responsibilities
  • Coordinate Patient Care: Work with patients, families, and healthcare teams to develop and implement individualized care plans.
  • Assess Patient Needs: Conduct thorough assessments to identify patient needs and preferences.
  • Develop Care Plans: Create and implement care plans that address patient needs and promote optimal outcomes.
  • Collaborate with Healthcare Teams: Work with interdisciplinary teams to ensure seamless care coordination and communication.
  • Provide Education and Support: Educate patients and families on health management and provide support to promote self-care and independence.
  • Monitor and Evaluate Care: Continuously monitor and evaluate patient care to ensure quality and effectiveness.
Requirements
  • Education: Bachelor of Science in Nursing from an accredited nursing program.
  • Experience: 1 year of registered nursing experience required, 3 years of related and relevant registered nursing experience strongly preferred.
  • Certification: Specialty Board Certification strongly preferred.
  • Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia.
Physical Demands

The job requires standing for prolonged periods, frequent travel, and bending/stooping. Proficient communicative, auditory, and visual skills are required, as well as attention to detail and the ability to write legibly. The ability to lift/push/pull lbs. may be required. May be exposed to chemicals, blood/body fluids, and infectious diseases.
