Position Overview:
The Nurse Care Coordinator (NCC) plays a pivotal role in orchestrating patient and family-centered care initiatives among various stakeholders. Utilizing advanced critical thinking skills, the NCC is tasked with crafting, executing, and assessing personalized care strategies aimed at empowering patients and enhancing health outcomes.

Key Responsibilities:
The NCC is responsible for the following:
Facilitating and coordinating resources and patient care activities to ensure effective transition management, collaborating across the healthcare continuum to meet the unique needs of patients and their families. Integrating professional nursing standards and ethical guidelines into practice, ensuring adherence to established protocols for care coordination and transition management. Conducting thorough assessments to identify patient risks and needs, tailoring care plans accordingly. Planning and prioritizing care activities in partnership with an interdisciplinary team, considering patients' individual preferences and desired outcomes. Ensuring safety and continuity of care through effective documentation and communication processes. Engaging with patients, families, and caregivers to support informed healthcare decisions, respecting cultural values and beliefs. Delivering health education and coaching, focusing on evidence-based practices to enhance patient understanding and self-management strategies. Leading initiatives that promote health equity and improve population health outcomes. Participating in research and applying evidence-based practices to enhance patient care across various stages of life. Overseeing the continuity of care, facilitating pre-visit coordination, post-visit follow-ups, and smooth transitions between services. Supporting medication management and addressing any related concerns. Other responsibilities as assigned.
Minimum Qualifications:
Education: Bachelor of Science in Nursing from an accredited institution. Experience: Minimum of 1 year of registered nursing experience; 3 years of relevant experience preferred. Certification: Specialty Board Certification is highly desirable. Licensure: Must hold a valid Registered Nurse license in the Commonwealth of Virginia and possess current BLS certification from the American Heart Association.
Physical Requirements:
The role involves prolonged standing, frequent travel, and physical activities such as bending and lifting. Strong communication skills and attention to detail are essential. The position may involve exposure to various health-related environments.
