Position Overview:
The Medication Care Coordinator plays a vital role in ensuring the delivery of exceptional care and services by administering medications and treatments in a secure and organized manner.

Key Responsibilities:
Responsibilities encompass the administration of medications, accurate documentation of medication distribution, conducting Service and Health Updates (SHUs), and providing comprehensive resident care while embodying the mission of Sunrise Senior Living, which is to enhance the quality of life for all seniors. This role adheres to all applicable federal, state, and local regulations, as well as Sunrise Senior Living policies, to ensure the highest standards of care.

Work Schedule:
Full-Time, 3PM-11PM

Core Duties:
As a member of the Sunrise team, it is essential to uphold our Mission, Principles of Service, and Core Values. Key duties include:

  • Delivering the highest quality of care by administering medications and treatments safely.
  • Reviewing and documenting pertinent resident information in the Daily Log.
  • Receiving medication updates from the Resident Care Director (RCD) or Wellness Nurse.
  • Administering and observing medications according to the medication administration record and the Six Rights of Medication Pass.
  • Maintaining confidentiality of resident information and reporting any concerns to the RCD or Wellness Nurse.
  • Assisting in medication checks and assessing residents for the need for 'as needed' medications.
  • Counting narcotics with another Medication Care Coordinator or Lead Care Manager at the beginning of each shift.

Resident Care:
Engaging with residents to promote independence and dignity while ensuring their physical safety. This includes:

  • Notifying the RCD of any resident or family concerns through the LEAD process.
  • Participating in the development and updates of Individualized Service Plans (ISP) and Individualized Care Plans (ICP).

Safety and Compliance:
Working collaboratively with the community team to ensure compliance with safety regulations and promoting risk management policies. Responsibilities include:

  • Practicing safety procedures, including the use of Personal Protective Equipment (PPE).
  • Reporting accidents and unsafe conditions immediately.
  • Ensuring that all hazards are managed appropriately and in compliance with fire codes.
  • Adhering to infection control techniques and reporting any occupational exposures promptly.

Team Collaboration:
Commitment to serving residents through our Principles of Service. This includes:

  • Participating in engagement programs and training sessions.
  • Maintaining compliance with required training and certifications, including CPR and First Aid.
  • Demonstrating a desire to serve and care for seniors while remaining calm in challenging situations.
