Job Overview

The ideal candidate will deliver a comprehensive suite of on-site, client-focused, strengths-oriented services to a manageable caseload of around 20-25 individuals, aiming to assist them in achieving meaningful lives within the community.

Key Responsibilities

The role encompasses a variety of duties, including but not limited to:

  • Conducting initial assessments to identify urgent needs and respond accordingly.
  • Utilizing assessment frameworks to pinpoint necessary support services and facilitate immediate referrals.
  • Introducing clients to the program, outlining requirements and potential outcomes.
  • Collaborating with other social service providers to create and implement an Independent Living Plan (ILP).
  • Assisting clients who are unemployed in accessing public assistance programs.
  • Completing psychosocial evaluations within the initial month of client intake.
  • Monitoring and documenting client progress regularly.
  • Encouraging clients to take an active role in their planning and goal-setting processes.
  • Holding regular meetings with clients to discuss their challenges and achievements.
  • Facilitating referrals for job development opportunities when suitable.
  • Connecting clients with appropriate resources to help them meet their objectives.
  • Engaging with clients to address their issues and enhance their capacity for long-term self-sufficiency.
  • Maintaining detailed client records, including interactions, warnings, and progress updates.
  • Ensuring confidentiality of all client information.
  • Reporting critical incidents promptly to the appropriate supervisory personnel.
  • Collaborating with housing specialists and employment staff to assess client readiness for employment and housing opportunities.
  • Upholding a standard of kindness, respect, and dignity in all interactions with clients, visitors, and colleagues.
  • Providing ongoing case management and counseling support for up to one year following a client's departure.
  • Fostering connections with other organizational programs to streamline inter-agency referrals.
Qualifications

To be considered for this position, candidates should possess:

  • A bachelor's degree in Social Work, Psychology, Sociology, or a related discipline with a minimum of 2 years of relevant experience.
  • An associate degree in Human Services or a related field with at least 4 years of relevant experience.
  • A high school diploma or equivalent with 8 or more years of direct social service experience.
  • Exceptional interpersonal skills, demonstrating compassion and firmness while maintaining confidentiality.
  • A solid understanding of community resources.
  • The ability to resolve conflicts, make informed decisions, and listen effectively.
  • Fluency in English; proficiency in Spanish is advantageous.
  • The capacity to remain calm in crisis situations.
  • A proven track record of collaborating effectively with diverse groups.
  • Strong organizational skills, attention to detail, and efficiency in task management.
  • Demonstrated maturity, integrity, and sound judgment.
