Key Responsibilities:
  • Provide rehabilitation, case management, and supportive services to individuals on the ACT Team
  • Facilitate vocational training, adult education, and employment programs
  • Evaluate and deliver direct assistance in areas such as health, housing, education, and medication management
  • Engage in treatment meetings and contribute to crisis intervention strategies
Required Qualifications:
