Position Overview:

The Surgery/Trauma Clinical Pharmacist plays a crucial role within a multidisciplinary healthcare team, delivering comprehensive pharmaceutical services to patients with high healthcare utilization at Vanderbilt University Medical Center. This position focuses on supporting trauma care teams and assisting nursing staff on surgical stepdown units. Responsibilities include participating in discharge meetings or patient rounds as necessary, establishing specific pharmacotherapeutic objectives, recommending treatment regimens, and developing monitoring strategies tailored to each patient's medication needs. The clinical pharmacist will also provide deprescribing recommendations and facilitate seamless transitions of care. Additionally, this role is responsible for leading initiatives aimed at preventing and reporting medication errors and adverse drug reactions, adhering to established policies, and striving for continuous improvement in medication safety practices.

Key Responsibilities:

  • Evaluates medication orders and prescriptions to ensure safe, effective, and economical medication use. Prepares and dispenses medications, offers drug information, and manages medication therapy in compliance with regulations and institutional policies.
  • Delivers exceptional patient care by optimizing medication management and resolving medication-related issues.
  • Ensures the quality, safety, and consistency of clinical pharmacy services in designated areas.
  • Oversees the activities of assigned support staff, verifying the accuracy and appropriateness of all medications prepared by them prior to dispensing.
  • The responsibilities outlined are a general overview of the position; additional duties may be assigned as needed.

Qualifications:

Preferred:

  • Completion of an ASHP-accredited PGY1 residency with additional relevant experience or an ASHP-accredited PGY2 residency (internal medicine, geriatrics, or similar).
  • Board of Pharmacy Specialties Certification in Geriatrics (BCGP) or Pharmacotherapy (BCPS).
  • Experience in an academic medical center setting.

Work Schedule:

  • Day Shift
  • Every 4th weekend
  • Holiday rotation
