Position Overview:

POSITION SUMMARY


The Clinical Pharmacist will play a vital role within a multidisciplinary healthcare team, delivering comprehensive pharmaceutical services to adult patients undergoing abdominal solid organ transplants at Vanderbilt University Medical Center.

The primary responsibilities will include supporting inpatient services for Kidney/Pancreas (Kidney, SPK, and PAK), Liver Transplant Surgery, and Hepatology.

Key clinical services encompass participation in interdisciplinary rounds, management of immunosuppression therapies, medication education, and ensuring accurate medication reconciliation during admission and discharge, as well as facilitating seamless transitions of care.

The clinical pharmacist will also lead initiatives aimed at preventing and reporting medication errors and adverse drug reactions, in accordance with established policies, while continuously striving to enhance medication safety and therapeutic practices within the institution.

Effective collaboration with pharmacy, nursing, and medical teams, alongside exceptional communication skills, are essential for success in this role.

PRIMARY RESPONSIBILITIES

  • Evaluates medication orders and prescriptions to ensure safe, appropriate, and cost-effective medication use. Prepares and dispenses medications, provides drug information, and manages medication therapy in compliance with regulations and hospital policies.
  • Delivers exemplary patient care by optimizing medication preparation and resolving medication-related issues.
  • Upholds the quality, safety, and consistency of clinical pharmacy services in designated areas.
  • Oversees the activities of assigned support staff, ensuring the accuracy and appropriateness of all medications prepared prior to dispensing.
