Position Overview:

POSITION SUMMARY


The Clinical Pharmacist plays a vital role within a multidisciplinary healthcare team, delivering comprehensive pharmaceutical services to adult patients undergoing abdominal solid organ transplants at Vanderbilt University Medical Center.

The primary responsibilities include supporting inpatient services related to Kidney/Pancreas, Liver Transplant Surgery, and Hepatology.

Key clinical services encompass participation in interdisciplinary rounds, management of immunosuppression therapy, medication education, and ensuring accurate medication reconciliation during admissions and discharges, as well as facilitating smooth transitions of care.

The clinical pharmacist is also tasked with leading initiatives aimed at preventing and reporting medication errors and adverse drug reactions, adhering to established policies, and striving for continuous enhancement of medication safety practices.

Collaboration with pharmacy, nursing, and medical teams, along with exceptional communication skills, are essential for success in this role.

PRIMARY RESPONSIBILITIES

  • Evaluates medication orders and prescriptions to ensure safe, effective, and economical medication use. Prepares and dispenses medications, provides drug information, and manages medication therapy in compliance with regulations and institutional policies.
  • Delivers exceptional patient care by optimizing medication management and resolving medication-related issues.
  • Upholds the quality, safety, and consistency of clinical pharmacy services in designated areas.
  • Oversees the activities of support personnel, ensuring the accuracy and appropriateness of all medications prepared prior to dispensing.
