Key Responsibilities- Conduct thorough patient assessments to identify clinical interventions and develop effective treatment plans.
- Collaborate with medical teams to make informed clinical recommendations and improve patient outcomes.
- Perform pharmacokinetic consults for various medications, including vancomycin and aminoglycosides, to ensure safe and effective use.
- Develop and implement TPN regimens to meet patient needs and optimize outcomes.
- Provide patient education and counseling on medication use and management.
- Document clinical interventions and patient progress effectively.
- Identify opportunities for process improvements and implement solutions to enhance department efficiency.
- Supervise and train pharmacy technician personnel to ensure high-quality patient care.
- Stay up-to-date on hospital policies and regulations to ensure compliance.
- Advanced Cardiac Life Support certification within 90 days of employment.
- Basic Cardiac Life Support certification within 90 days of employment.
- TN Pharmacist licensure or eligibility.
- Bachelor's or Doctoral degree in Pharmacy.
- Previous clinical hospital experience and PGY1 residency preferred.
