Position Overview:

The Surgery/Trauma Clinical Pharmacist plays a crucial role within a multidisciplinary healthcare team, delivering comprehensive pharmaceutical services to patients with high healthcare needs at Vanderbilt University Medical Center. This role primarily involves supporting trauma-focused patient teams and assisting nursing staff on surgical stepdown units. Key clinical responsibilities include participating in discharge meetings or patient rounds as necessary, establishing individualized pharmacotherapeutic objectives, recommending treatment regimens, and developing monitoring strategies tailored to each patient's medication therapy requirements. Additionally, the clinical pharmacist will provide deprescribing recommendations and facilitate seamless transitions of care. The successful candidate will lead initiatives aimed at preventing and reporting medication errors and adverse drug reactions in accordance with established policies, continuously striving to enhance medication therapy safety within the institution. Responsibilities also encompass targeted drug therapy monitoring, conducting medication use evaluations, and engaging in quality improvement projects aligned with the goals of Vanderbilt University Medical Center. Strong teaching abilities are essential, as the clinical pharmacist will mentor pharmacy students and residents, and provide both formal and informal training to nursing and medical staff. Effective collaboration with pharmacy, nursing, and medical teams, along with exceptional communication skills, is vital.

Key Responsibilities:

  • Examine drug orders and prescriptions to ensure safe, appropriate, and cost-effective medication use. Prepare and dispense medications, provide drug information, and manage medication therapy in compliance with regulations and hospital policies.
  • Deliver exceptional patient care by optimizing medication preparation and resolving medication-related issues.
  • Maintain the quality, safety, and consistency of clinical pharmacy services in designated areas.
  • Oversee the activities of assigned support staff, ensuring the accuracy and appropriateness of all medications prepared prior to dispensing.
  • Additional responsibilities may be assigned as needed.

Qualifications:

Preferred:

  • Completion of an ASHP-accredited PGY1 residency plus over a year of relevant experience, or completion of an ASHP-accredited PGY2 residency (internal medicine, geriatrics, or similar).
  • Board of Pharmacy Specialties Board Certification in Geriatrics (BCGP) or Pharmacotherapy (BCPS).
  • Experience in an academic medical center setting.

Work Schedule:
