Job Title: Lead Nurse Coordinator
Location: Sobering Center

About Horizon Services, Inc.:
Established in 1974, Horizon Services, Inc. (HSI) is a nonprofit organization dedicated to providing addiction treatment aimed at alleviating substance use-related challenges for individuals from diverse backgrounds, including marginalized communities. Our core values of Integrity, Compassion, Respect, and Professionalism guide our mission to deliver residential treatment, withdrawal management, and sobering programs, supporting the well-being of women, men, youth, LGBTQ, and BIPOC clients to lead healthy and fulfilling lives. HSI fosters an inclusive workplace and a learning community where collective experiences contribute to mutual growth.

Role Responsibilities:
The Lead Nurse Coordinator plays a crucial role in assessing participants for suitability to enter the Sobering Center, focusing on current and potential medical issues, mental health conditions, and withdrawal symptoms. This position involves referring clients for comprehensive medical and mental health care when necessary.

Impact on Lives:
In this capacity, you will significantly influence the lives of individuals facing substance abuse challenges, often accompanied by co-occurring conditions. You will enhance your skills in compassionate care, crisis intervention, and de-escalation within a supportive and inclusive environment. This role provides the opportunity to offer substance abuse treatment to individuals embarking on their recovery journey, transforming hopelessness into a sense of value and potential for a better future.

Key Responsibilities:

  • Conduct health screenings for all prospective clients.
  • Monitor vital signs, perform medical triage, and assess clients' needs for sobering.
  • Maintain accurate records of clients' medications in a logbook or electronic health record (EHR) system.
  • Execute appropriate assessments, including rapid risk intoxication evaluations and vital checks for each client entering the recovery station.
  • Document observations every 30 minutes and case notes every 6 hours in the EHR, covering medication observations, case management, crisis interventions, and discharge processes.
  • Create a welcoming atmosphere for participants.
  • Implement conflict resolution strategies and de-escalate situations with hostile clients.
  • Respond to crises, emergencies, and other client needs promptly and appropriately.

Team Collaboration:
Work collaboratively as part of a multidisciplinary team at the Recovery Station, participating in daily shift changes and reporting health-related concerns. Support the maintenance of a clean and safe environment, serve as shift lead, and assist other health technicians as needed. Assume management responsibilities in the absence of the program manager.

Quality Assurance:
Monitor and maintain client records in the EHR for accuracy, ensuring all risk assessments and medication clearances are properly documented and uploaded into the client chart daily.

Qualifications:

  • Current Licensed Vocational Nurse (LVN).
  • Demonstrated knowledge of Substance Use Disorders, Mental Health, and Crisis Intervention.
  • Minimum of 1 year of experience in a similar setting with proven leadership capabilities.
  • CPR/First Aid certification.
  • Valid California driver's license with a good driving record.

Compensation and Benefits:
We offer a comprehensive benefits package for all full-time and part-time employees working over 20 hours per week, including:

  • 100% coverage of medical, dental, life, and long-term disability insurance for employees, with 50% coverage for dependents.
  • Paid time off (PTO) and holiday pay.
  • Retirement benefits after 6 months of service.
  • Training and continuing education opportunities.

Salary: $35-$40.00 per hour, commensurate with experience and qualifications.
